Abstract: At approximately 12:10 p.m. on October 5, 1998, Employee #1, of Sierra Pacific Industries, was working as part of a four-man crew that was lowering a section of an ash chute from the bottom of a co-generation plant to the ground. The approximately 20 ft long by 4 ft tall by 2 1/2 ft wide section was concrete-lined and weighed about 12,000 lb. The crew had welded pieces of steel I-beams across the bottom about 3 ft from each end and attached chain hoists to the I-beams at each end before removing all the bolts from the chute. A coworker was working the chain hoist from above and Employee #1 was standing on one of the I-beams. As they started to lower the chute and clear the bolts, the top of the chute suddenly swung sideways against the hoist chain, catching Employee #1's right little finger between them and striking his upper torso. His finger was amputated and he sustained various internal compression injuries. Employee #1 was flown by Life Flight to Washoe Hospital in Reno, NV, where he was admitted for treatment.
