Abstract: At approximately 5:15 p.m. on September 10, 1998, Employee #1, of Camblin Steel Service, was riding on the running board of a forklift when he started to climb up onto the bed of a truck trailer that was being loaded in the yard. He placed his hand on a cross member of the mast and stepped onto the front fender as the forklift operator lowered a bundle of rebar onto the trailer. As the mast sections of the forklift passed by each other, Employee #1's fingers became caught in the shear point, amputating his right index finger and lacerating several others. He received first aid from the local fire department before being transported by ambulance to UC/Davis Medical Center.
