Abstract: At approximately 1:45 p.m. on December 7, 1993, Employee #1 was installing a large, 14 foot long hydraulic speed shore in a 16 foot deep excavation. He had completed the pump action associated with the hydraulic shoring system and was disconnecting the quick release hose connection when the shore collapsed and scissored to the bottom of the excavation. Employee #1 had been resting one foot on the speed shore's horizontal hydraulic ram, and when the shore device collapsed, he fell into the excavation, injuring himself. He was hospitalized for his injuries. There were no malfunctions or discrepancies associated with the speed shore device. After the accident, a coworker discovered both pump valves and the shore open, indicating that the hydraulic pressure bled off from the shore device to the pump tank. Employee #1 was trained in the installation of shore devices. No violation of T8 standards was found.
