Abstract: On August 29, 1996, Employee #1 was using a John Deere 664G front end loader to carry some 4 ft by 12 ft crane outrigger pads. Starting at the north side of the Central Freeway project, he would pick up two pads in the jaws of the clamshell-type bucket and carry them the 3 mile length of the freeway site to a drop area. After eight complete runs, Employee #1 was in the middle of a ninth run, approximately 200 ft from drop site, when he dropped the pads. The loader ran over them and Employee #1 was thrown from the cab and killed. He was not wearing a seat belt and the loader's cab door was locked open.
