Abstract: At 2:30 p.m. on June 26, 1995, Employee #1 and his foreman were dumping chips out of the dump bed of an elevating boom truck that they were using during a tree trimming operation. Employee #1 climbed onto the boom truck to raise the boom, which in turn would raise the dump bed. After the boom was raised, and before Employee #1 descended, the foreman began to move the truck to spread the wood chips in the company's storage yard. Employee #1 lost his balance and fell. His feet and ankles were run over by the truck's rear wheels. Employee #1 sustained a large bump between his eyes, facial abrasions, abrasions and lacerations on his legs below the knees, fractures to both ankles, and crushed a foot.
