Abstract: At 4:30 p.m., Employee #1 was crossing a street at his workplace. An unloaded forklift came from his right and struck him as he was walking in the marked crosswalk. Employee #1 was killed. There were no other vehicles or pedestrians were on the street, and Employee #1 and the forklift operator apparently did not see one another. The forklift operator later stated that he heard someone yell, looked up, applied the brakes, and saw Employee #1 just as the forklift struck him. The operator estimated his speed at 5 to 10 mph; a radar speed test showed the maximum speed of the forklift to be 12 mph. The operator said he was looking right and left, although a witness said he only saw the operator look to his right. The cause of the accident appeared to be human error on the part of both the operator and Employee #1.
