THE EMPLOYEE WAS FOUND LYING ON THE FLOOR WITH THE POWER WASHER WAND NEXT TO HIM. THE WASHER HAD BEEN PLUGGED INTO A GROUNDED DROP LIGHT CORD SET WITH A RECEPTACLE ON IT.
