Abstract: At about 2:30 p.m. on August 26, 1991, Employee #1, of General Temporary Services, started to clean the blade of a pie dough cutter by removing the top cover. Employee #1 stated that the power to the machine was off. There was no eyewitness; therefore it is presumed that Employee #1 accidentally turned on the nearby disconnect switch with his clothing. There is also a micro switch on the machine that is in the OFF position when the cover is removed. Employee #1 caught his left ring finger in the blade of the pie dough cutter, sustaining a compound fracture. He was rushed to Presbyterian Intercommunity Hospital in Whittier for emergency repair of his ring finger, which was repaired; he was then discharged. According to the bakery plant manager, the micro switch was found pushed down with a piece of a paper during an inspection after the accident. It is not known how long this paper had been bypassing the switch. The employer was cited for a general violation of 8CCR 3314(a).
