Abstract: Employee #1, age 18, was on a crew of three that was cleaning a cereal machine when water seeped into the separator chute. The machine was turned off and the water was cleaned from the chute area, which housed five rotary blades. A coworker went to turn on the machine, which had a 5 second start delay when activated. Employee #1, seeing some water left in the chute, stuck his hand in to wipe it up. The machine activated and his middle finger was severed.
