Abstract: AN EMPLOYEE OF AN ELECTRIC AND WATER UTILITY WAS INSERTING A LOCKING PIN INTO POSITION ON A ROTATING DRIVE SHAFT AND SPROCKET. HIS LEFT RING FINGER WAS AMPUTATED WHEN IT GOT CAUGHT BETWEEN THE METAL HOUSING AND THE LOCKING PIN.
