Abstract: At approximately 5:35 p.m. on April 23, 1991, Employee #1 and the machine operator were making a routine adjustment on a clamp assembly machine. The machine had been in the RUN position for a short time, and Employee #1 told the operator to stop the machine so he could check the adjustment. Employee #1 was engaged in conversation with another coworker when the machine operator heard him say "OK," and thought that Employee #1 had completed the adjustments. The operator activated the machine, causing the last stake to crush and lacerate Employee #1's left index finger. He was transported him to St. Rose Hospital for treatment. On April 24, 1991, we was transferred to Merritt Hospital in Oakland, where he underwent debridement and suturing by a Board-certified hand surgeon.
