Abstract: At 9:10 a.m., Employee #1, a distributor operator on the front cocktail dicers, had just returned from morning break when she discovered that she had left her gloves in the break room. As she hurried along the runway for the dicing machines, she saw a friend working on the floor. As she and her friend waved to each other, she took a wrong turn on the dicing platform and struck an access gate consisting only of one 24 in. by 2 in. by 1/4 in. stainless steel bar. The bar gave way and she fell 12 ft onto the concrete floor, sustaining injuries that required hospitalization. Although she had worked as a distributor operator for about 5 of her 12 years at the Mart, she had worked in this area for only 2 weeks.
