Abstract: At approximately 2:00 p.m. on October 17, 1991, Employee #1, of LP Construction, was installing a water line in Healdsburg, CA. He was in a 7 ft by 3 ft by 7 ft deep trench that had been dug the previous day to expose a portion of the old water line that was to be removed. Employee #1 was using a power saw to cut the lateral pipe when the trench caved in on him. He sustained serious injuries and was transported by ambulance to Healdsburg General Hospital; he was later transferred to Santa Rosa Memorial Hospital. Nearly 2 months earlier, the city had faxed a memo to LP Construction about the lack of shoring in a trench over 5 ft deep. Employee #1 had worked in underground construction for 6 years and for LP Construction for 2 months.
