Abstract: Employee #1 was driving a forklift backwards with a load on the forks while looking over his right shoulder. There are two versions of the accident: Employee #1's and a witness's. One is that a coworker was backing a truck from inside a cargo truck and crashed into Employee #1's forklift, striking his left foot, which was outside the running lines of the forklift. The other is that Employee #1 was backing up his truck when the coworker bumped him from behind. Employee #1 stopped and the coworker pulled his forklift so closely parallel to Employee #1 that Employee #1's foot, which was sticking outside of his forklift, was caught by the body of the other forklift and crushed. Three toes were amputated on Employee #1's left foot.
