Abstract: At approximately 9:05 a.m. on July 3, 1992, Employee #1, the owner of the company, went to clear a piece of paper from a Sakupai silk screening machine. While the machine was running, Employee #1 extended his body to reach with his right hand and inserted it into the machine's point of operation. Employee #1's ring and middle fingers were amputated. He was taken to White Memorial Hospital, where he was treated and released. Employee #1 was given information on the hazards of operating a silk screening machine and on safe practices to prevent similar accidents.
