Abstract: Employee #1, a foreman for Top Stitch, was using a forklift to move some pallets. One of the pallets broke and a part of the wood lodged in the chain that operates the truck's lifting mechanism. Employee #1 knelt down and stuck his arm up behind the frame guard, which prevents contact with the lifting mechanism. When he dislodged the piece of wood, the mechanism shot upward, trapping and crushing the employee's arm. Employee #1 was transported to Beverly Hospital in Montebello, CA, where all five fingers and half of his hand were surgically amputated.
