Abstract: At approximately 6:40 p.m. on January 26, 1995, Employee #1, of Sequoia Wood Products, Inc., was operating a Powermatic table saw, model 66, serial #916659160, with a 10 in. blade. He was cutting 1/8 in. off both ends of 8 in. by 13 in. pieces and had been using a guide box, but found he was unable to make even cuts, so he stopped using the guide. He had cut approximately 50 pieces when one piece bound up in the saw blade, causing his right hand to contact the unguarded blade. His index finger was amputated below the first joint. He also suffered nerve damage with a deep laceration to his middle finger, and required a skin graft on his thumb. Employee #1 was hospitalized overnight at Pacifica Hospital.
