Abstract: Employee #1 was installing aluminum siding on a building's fascia from an approximately 30 foot high scaffold. The scaffold consisted of one approximately 30 ft long aluminum pole on the side of the building, one aluminum scaffold plank, and one 12 ft aluminum ladder placed on the porch roof that was used to support the scaffold plank, which ran from the ladder to the pole. The ladder was tied off by nailing a piece of aluminum siding to the fascia that was attached to the ladder. Employee #1 went to move the ladder, as it was in his way. While standing on the scaffold plank, he took a hammer and knocked out an aluminum strip that was securing the ladder in place. The strip came out, the ladder started to move, and Employee #1 grabbed onto it. The ladder continued to move, and the employee held on. Both the ladder and the employee fell to the ground. Employee #1 sustained a skull fracture. He died.
