Abstract: On August 26, 2006, Employees #1 through #5 were pulling conductors in a main electrical panel box. They had removed three of the breaker boxes from the energized panel when something contacted the busbars. This shorted them out and caused an arc flash and explosion. Employees #1, #2, and #3 sustained critical burns and were flown to the Wishard Hospital Burn Unit in Indianapolis, IN. Employee #4 was treated at the Bloomington Hospital and then released. Employee #5 was treated at the scene. On October 13, 2006, Employee #2 died while still a patient at the burn unit.
