Abstract: At approximately 12:00 noon on May 20, 1996, Employee #1, a truck driver, was delivering a tanker load of Jaylex to an address in Everett, WA. His PTO-driven pump was not working or was contaminated so he called the company dispatcher, who sent another driver to off load Employee #1's truck. Employee #1 unhooked his trailer, got into his tractor, and pulled it ahead. He was then supposed to back out, leaving room for the other driver to hook up and pump out the trailer. After a short time, the second driver heard the brakes set on Employee #1's tractor, and went to find out if there was a problem. He found Employee #1 collapsed in the driver's seat, gasping for air. An employee of Achilles USA, Inc., was passing by and the second driver had him stay with Employee #1 while he went to call 911 from his truck's cell phone. He then went to the guard shack to direct emergency personnel to the incident site. Their response time was 3 to 5 minutes. Medical assistance and CPR were administered to Employee #1 for 30 to 40 minutes, without success. Everett police stayed at the site until the Snohomish County Coroner's Office arrived to transport Employee #1's body. He died of a heart attack and there was no autopsy.
