Abstract: At 5:45 p.m. on June 7, 2024, Employee #1 was working on an active highway construction project, conducting inspections, measurements, and progress checks in various work zones. Employee #1 entered a work zone where a CAT grader (Model: 120, Build: 14B, Product ID: CAT00120JY9A00806, Weight: 44,093 lbs.) was operating. After making verbal and visual contact with the Grader Operator, Employee #1 performed the tasks and left the work zone. Later that evening, Employee #1 re-entered the active work zone without making verbal or visual contact with the Grader Operator. The employee unknowingly entered the path of the moving grader and was struck by the left front tire of the 44,093-pound grader, resulting in fatal crushing injuries.
