Abstract: At 3:00 p.m. on January 3, 2012, Employee #1 was setting a chain fall to erect steel beams. Employee #1 was relocating the rigging to lift another beam into place. Employee #1 was carrying a ladder to this location, when he fell through an unprotected floor hole. Employee #1 fell approximately 22 feet to the concrete ground causing severe injuries to his left hip and leg. Employee #1 was hospitalized and treated for his injuries.
