Abstract: At 6:23 p.m. on April 19, 2023, an employee was in the process of unhooking chains attached to a metal plate that was over a 6-foot wide by 100-foot deep hole. The employee fell down a dry well approximately 100 feet deep. The employee was killed. At the time this file was submitted for review, the medical examiner's report had not been received.
