Abstract: At 4:34 a.m. on February 28, 2023, an employee working for an equipment and machinery manufacturer was sitting at a lunchroom table. The employee had started her work shift at 5:00 p.m. and worked as a confined space attendant until lunch time, which was from 11:00 p.m. to 11:45 p.m. The lunchroom was used as a staging/resting area for confined space attendants when they weren't performing attendant duties. The employee never returned to work after lunch and was seen going to her car at approximately 12:38 a.m. She returned (scanned in) to the building at 12:51 a.m. When the returned to the lunchroom, she was observed with her arm on the table with her head resting on her arm. At the end of the work shift when it was time to leave the facility, a coworker tapped the employee on the shoulder in an attempt to wake her up. The employee was unresponsive and had died of an unknown illness.
