Abstract: EMPLOYEE #1 WAS BUILDING A CONCRETE FORM DECK OVER AN OPEN STAIRWAY. HE LOST HIS FOOTING AND FELL 67 FEET TO THE CONCRETE FLOOR. THERE WAS NO TEMPORARY FLOOR, SAFETY NET, OR LIFELINE AND SAFETY BELT IN USE.
