SERIAL # FWV436901, LICENSE #WVC39-844), FOR TRANSPORATION TO THE COMPANY SHOP. WHILE BEING DRIVEN ONTO THE LOWBOY, THE BULLDOZER OVERTURNED TO THE REAR. EMPLOYEE #1 WAS PINNED UNDERNEATH THE BULLDOZER AND FATALLY CRUSHED.
