Abstract: Between approximately 1:00 and 2:00 p.m. on Saturday, September 4, 1993, Employee #1 left the animal feed manufacturing and sales site where he worked. At about 3:30 p.m., an employee of a contract fumigating company started to fumigate the worksite using a pesticide containing aluminum phosphide, which reacts with water to form the toxic gas, phosphine. He completed this task at approximately 4:00 p.m., locked the doors, and posted placards warning of the hazard. At some point after the fumigator left, but well before the safe entry date listed on the placards, Employee #1 entered the worksite. His body was discovered inside the locked building at approximately 1:30 p.m. on Monday, September 6, 1993.
