Abstract: Employee #1 was dispatched to a tractor supply company to repair a forklift truck that was stuck in the up position, holding a pallet of salt about 5 ft above the floor. Employee #1 used a 0.25- or 0.5-in. chain with a hook to secure the mast to the frame, and a screwdriver to block the control in the down position. He then loosened the feeder tube valve while under the loaded truck. This released the hydraulic pressure from the cylinder, and, with the control blocked in the down position, put all the load's weight on the chain. The chain failed and the 1,250-lb load fell onto Employee #1, killing him.
