Abstract: On March 7, 1991, Employee #1 was at the top of a 16 ft high tubular scaffold installing electrical connectors in the overhead lights at the four new gasoline bays at L & G Auto Truck Plaza. The scaffold had been rented after lunch and had been brought to the site and set up at about 1:00 p.m. From 1:20 p.m. to 2:20 p.m., Employee #1 had been riding the scaffold as a coworker pushed it from light to light. There were sheets of sheet metal lying on the concrete and PVC pipe and a 3 in. high by 6 in. wide pile of dirt and broken pieces of concrete at the south end of the fourth bay. The coworker had gone through the pile of dirt once with the scaffold while Employee #1 was aboard. He had gone to the other three bays and was turning to go into the last bay, intending to follow the same tracks through the dirt pile. As the front wheels of the scaffold passed through the dirt pile, the scaffold wheel hit a piece of concrete. The end of the scaffold started to flip over, and the coworker tried to stop it, but he could not. Employee #1, who was holding the railing on the end of the scaffold and had his back to the railing, rode the scaffold down. He landed on the concrete with the scaffold railing landing about his waist and his head hitting the concrete. He died five days later, at 12:30 p.m. on March 12, 1991.
