Abstract: Employee #1 was enlarging an existing hole in the top of a manhole cover when the diamond segmented chain on a Maxcut 12 saw broke and projected a diamond segment into his chest. The segment fractured his sternum and severed his aorta. Employee #1 died. The pneumatic powered concrete saw was manufactured by Spitnas, type 510090000, Masch. NR. 914141.
