Abstract: Employee #1 was operating a cookie machine. The machine contains a mold plate that slides horizontally in and out of the machine. The mold plate contains two rows of circular slots that are filled with cookie dough; a plate containing two rows of cylindrical projections (knockouts) is situated above the mold plate. The plate descends vertically and pushes the dough out of each circular slot in the mold plate and onto a conveyor. Employee #1 reached under an interlocked guard and put his fingers up through a circular slot in the mold plate to clean the knockout. The mold plate retracted and the employee's fingers were severed when they caught between the edge of a circular slot on the mold plate and the frame of the cookie machine.
