Abstract: EMPLOYEE #1 CLIMBED ONTO THE CHIP JAW OF AN AUTOMATIC LATHE. WHILE HE WAS CHANGING THE CUTTING TOOL TIPS THE PRODUCT HANDLING ROBOT MOVED. HE WAS CRUSHED AGAINST THE HEAD STOCK OF THE LATHE.
