Abstract: Employee #1 was delivering a 17,736 pound load of handrails and I-beams to a construction site. He was driving a Peterbilt truck, vin #KD 283495, and towing flatbed trailer #26, which belonged to W.W. Steel Co. of Oklahoma City, OK. When Employee #1 arrived at the site, he was met by two employees of the construction company. One of the employees rode in the truck with Employee #1 to a staging area; the other followed in a forklift. At the staging area, the forklift driver told Employee #1 that his load was leaning to the driver's side of the truck and not to undo the tie-down straps until the forklift was positioned against the load. The forklift driver began to position the forklift when the other employee yelled that the forklift tire was almost flat. The forklift operator told Employee #1 again not to undo the tie downs, and that he would get air and be back. While he was gone, Employee #1 loosened the tie-down straps. Part of the load fell off the trailer and crushed him to death.
