Abstract: At 4:15 a.m. on April 13, 1993, Employee #1, an employee of Jeld Wen Fiber of Klamath Falls, Oregon, had the tip of his right middle finger amputated when he reached into an inspection door to remove plugged material from a fiber/resin blender. He was performing cleanup and placed his hand inside the inspection door while the high speed blender was in operation. The machine had not been shut down and/or locked out. His supervisor was helping with the cleanup. Employee #1 was transported to Merle West Medical Center in Klamath Falls and was released later that day.
