Abstract: At 12:27 p.m. on February 24, 2021, Employee #1 was working for a plumbing/HVAC contractor. The worksite is a Warehouse Distribution Center. Employee was installing new duct work in the facility. Employee #1 was working from a scissor lift that was being lifted by a forklift. The scissor lift fell from the elevated forks causing Employee #1 to strike the ground fracturing his jaw. Employee #1 was hospitalized.
