Abstract: At 12:00 p.m. on June 5, 2020, Employee #1 arrived at Caliber Collision to conduct a pest (rats) control survey. Employee #1 decided to look at the ceiling of the inner offices to determine if he could find entry points the rats could use to enter the offices. To access the ceiling, Employee #1 climbed a fixed ladder attached to the outside wall of the inner offices, which stood approximately 12 to 14 feet above the concrete floor. Employee #1 climbed the ladder and decided to walk the ceiling to see if he could find an entry point. Employee #1 stepped onto the ceiling and moved six feet to the wall on the left. As Employee #1 moved to the left, his left foot went into a hole, and he fell through the ceiling onto a desk and then to the floor. Employee #1 was hospitalized and underwent back surgery for a broken back. T8CCR 3203(a): The employer failed to effectively implement the company's Injury and Illness Prevention Program (IIPP). The employer failed to: 1. Effectively implement the company's hazard assessment standards, addressed in the company's IIPP, as the company performed a pest control survey at Caliber Collision in Gardena, CA. 2. Effectively implement the use of the company's IIPP designed communication system to train and inform employees on the hazard analysis for work to be done on elevated locations: such as inner office ceilings.
