Abstract: At approximately 3:00 p.m. on February 23, 2021, Employee #1 was operating an inverted table saw with a radial saw blade (Manufacturer: Robbins Machine Company, Model: The Hurricane, Serial Number: FE-090397, equipped with an 18-inch diameter, 36-tooth blade). While wrapping up work and cleaning at the end of the shift as instructed, the employee opened the secured front metal panel of the saw's enclosure using a hexagonal wrench. With the saw blade still running and exposed, the employee placed his hand inside the enclosure to scoop out wood dust. The employee's right hand came into contact with the running saw blade, resulting in the amputation of the tip of the right-hand ring finger (up to the knuckle) and a laceration to the right-hand middle finger. The employee was hospitalized and received medical treatment, including surgical amputation. The incident highlights the importance of proper lockout/tagout procedures and ensuring equipment is deenergized before performing maintenance or cleaning tasks.
