Abstract: At 7:57 p.m. on September 27, 2020, an employee was in the process of sanitizing and cleaning a RapidPak (Model: RP-55, Serial Number: 855203) and a Drake (Model: 1800, Serial Number: 03-503). Both pieces of equipment were in line with each other. The employee placed a lockout lock on the Drake, but did not place a lockout lock on the Rapidpak as he was waiting for a replacement. The employee was placing a blue combo bag over the forming heads before sanitation could begin. While placing the bag, the employee's hand triggered the photo eye for the forming head, the forming head cycled and pinched the employee. The employee amputated the middle finger and was treated without hospitalization.
