Abstract: At 4:15 a.m. on September 2, 2020, Employee # 1 was filling up fuel cans at a fuel station with a forklift. He fuel cans on the body of the forklift and then went to reach for the cans. He accidentally activated the forklift boom lever, lowering the boom on top of himself, pinning him to the truck. Employee # 1 was pinned and killed.
