Abstract: At 1:45 p.m. on September 4, 2020, an employee was loading a 4 foot by 8 foot steel plate, 3/4 inches thick, onto a flatbed truck with a Mitsubishi forklift. When the employee released the plate from a clamp, it fell toward him, knocked him off the truck, and fell on top of him. The employee was transported to the hospital, where he later died due to crushing injuries.
