Abstract: At 3:50 p.m. on December 3, 2019, an employee and a coworker were working on Dock #76. The coworker was trying to move a stack of plastic (45 pallets, 16 pounds each pallet) with a forklift (Yale, Model: Veracitor 50VX, GLC050VXNVSE085, Serial Number:A910V06664D, Capacity: 4800 lbs.). As he lifted up the load with forks and tried to turn towards the dock, the stack of pallet got stuck in a rolling container (OTR), causing the stack of pallets, which was not secured, to tip over and fall onto the employee who was walking by. The unsecured stack of pallets caused the injury. The employee sustained a fracture on his hip and laceration on his head and left knee. The employee was transported to Arrowhead Regional Medical Center, where he was hospitalized for two weeks, and a surgery was performed on his hip. The site is a fixed facility warehouse, supplying boxes for U.S. Postal Services.
