Abstract: At 12:30 p.m. on November 20, 2019, an employee, a lead road technician, was preparing a Crown RR 5700 Series Narrow Aisle Reach Truck, Model: RR5725-45 , Serial Number: 1A428168, for transport by securing the mast using steel banding. He was standing on a storage pallet that was placed on the forks of another Crown SP-3500 Series Stock Picker, Model: SP3520-30, Serial Number: 1A495177 , which was elevated approximately 8-feet above ground, so that he could work on the first reach truck. The employee was using crimping pliers to crimp a clamp onto the steel banding. The employee was in the process of crimping a clamp onto the steel banding using crimping pliers, when the pliers slipped off of the clamp. This caused the employee to lose his balance and fall from the elevated storage pallet, 8-feet 3-inches to the ground below. The employee sustained a fractured left wrist and was hospitalized to treat fractures to the L2 and L3 lumbar cervical spine.
