Abstract: At 11:00 a.m. on June 14, 2019, Employee #1, employed by an HVAC company, was installing plenums on the roof of a one-story commercial building. He fell through a plenum opening, a fall height of 16 feet, and struck the concrete floor. The employee was transported to the hospital and admitted for treatment of fractures to both wrists and multiple ribs, and a laceration to the head.
