Abstract: At approximately 5:00 p.m. on April 19, 2019, an employee walked upstairs from a PLC control area to a mezzanine level, prepared to take an ingredient sample from a maltodextrin radial feeder. The employee had to rotate a spindle cover 180 degrees from top-to-bottom to access an opening in a maltodextrin feeder which would facilitate sample-depositing into a large plastic bag. As the employee performed this challenging task due to her short stature, the thumb of a glove was caught by a slow-moving rotating spindle, causing a partial amputation. The in-running nip point was not guarded. The opening was approximately 4 in. wide by 12 in. long. The employee was hospitalized for these injuries.
