Abstract: At approximately 9:30 a.m. on April 25, 2019, an employee was standing at the conveyor control station and watching and straightening filled packages of cookies after they passed through a 90-degree curve in a slat conveyor before going through a metal detector and labeler. Packages had to pass through the labeler in the proper orientation. The curved area of the conveyor caused them to misalign at times. The employee reached with both gloved hands to straighten a package and her left little finger was caught in the space between two plastic slats on the conveyor and pulled into an opening between the end of the moving conveyor belt and stainless steel guard plate, resulting in the amputation of the tip of the finger. Surgery was performed but hospitalization was not required.
