Abstract: At 6:45 a.m. on April 17, 2019, an employee was working at a sawmill. He noticed that the conveyor under the trimmer saw was not functioning. He went to the rear of the trimmer and opened the cover to one of the blade areas. The employee leaned in toward the machine to check for a jam. His clothing became caught on an unguarded sprocket on the rear of the trimmer saw. His left arm was pulled into the nip point. He suffered an injury to his left arm that resulted in an amputation. He was hospitalized. There were two inspections associated with this investigation, 1394677 and 1394706.
