Abstract: At 4:50 p.m. on February 21, 2019, Employee #1 set up a bending machine to bend rebar. After activating the machine, his left hand was on the rebar while it was being bent. The employee's glove became caught by the rebar and his hand was pulled into the machine. Employee #1's thumb was pinched by the rebar and bending rod, and the thumb tip was amputated. The employee was hospitalized for treatment.
