Abstract: At approximately 12:00 p.m. on May 30, 2018, Employee #1 was working as a sorter, pushing potatoes at the south end of the head pulley of the Ernest conveyor, labeled C-24. The potatoes fall from the end of the Ernest conveyor onto a lower second conveyor, the draper dewater chain conveyor, labeled N-24. The draper conveyor consists of three foot long metal bars separated an inch apart and attached to two inch wide conveyor belts at either end. A metal delivery board was attached at the end of the Ernest conveyor frame so the potatoes can slide down into the draper conveyor. The delivery board extended approximately five inches and ran the length of the three foot conveyor. The weld on the delivery board, where it attached to the frame of the conveyor belt on the south side, had broken and the board was loose. Employee #1 had noticed it the day before but had not notified anyone. The potatoes began piling up on the lower draper conveyor at the end of the tail pulley because that conveyor had not been turned on over the delivery board. The employee was wearing cotton gloves with latex gloves over them. A coworker, another sorter, turned on the Ernest conveyor that was positioned further up the line near the tail pulley. Another coworker, a boxing employee, turned on the draper conveyor and did not give Employee #1 any warning. The employees hand slipped into the pinch point between the loose delivery board and the draper conveyor. The tip of Employee #1's right index finger was crushed at the pinch point, which resulted in the medical amputation of her right index finger at the distal phalanx. The employee was hospitalized for treatment.
