Abstract: At 8:40 p.m. on August 28, 2018, Employee #1, employed by a store interior design, manufacture, and installation company, was operating the Cincinnati PB-4 hydraulic press brake to form sheet metal parts that had two sequential bendings. The parts were approximately 3 inches wide and 48 inches long. The machine was protected by a light curtain to prevent proximity to the die gap which could be as wide as 7 inches. The mute point was typically set at 0.25 inches. Employee #1 increased the mute point on the downward stroke to increase working speed. His left thumb was partially crushed and amputated, with loss of seventy five percent of the nail. He was hospitalized.
