Abstract: At 4:00 p.m. on April 13, 2018, Employee #1, employed by a construction company, was working at a residential construction site. He was standing on wood forms and pouring concrete when he fell and landed on a metal rebar stake that was holding a wood form in place. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for a puncture wound to the buttocks.
